Submit change of address request in writing with signature of owner(s). If the title to the property is held jointly, both owners must sign. Please be sure to include all owner numbers. Address changes will be implemented in the system as soon as possible upon receipt.

Submit a photocopy of marriage certificate, divorce decree reinstating maiden name or other document effecting name change. Name changes will be implemented in the system as soon as possible upon receipt.
COMPANY OR CORPORATION NAME CHANGE
Submit the Certificate of Name Change that has been filed in the County or Parish where the property is located.
COMPANY OR CORPORATION MERGES
Submit a Certificate of Merger that has been filed in the County or Parish where the property is located. Please include the new tax identification number if applicable.
APPOINTMENT OF GUARDIAN OR POWER OF ATTORNEY
Submit a copy of Power of Attorney or Letters of Guardianship.
